To apply during the opening period, pick up an application at the parking lot of the St. John the Baptist Romanian Orthodox Church, located at 501 E School St., Woonsocket, RI 02895.

Once the application has been completed, it must be mailed to 679 Social Street, Woonsocket, RI 02895, from September 19, 2016 until October 3, 2016. Applications postmarked outside of those dates will not be accepted. The WHA recommends returning the application using Certified Mail, Return Receipt
